A case report of niacin-associated acute hepatotoxicity describes a patient who required an emergency liver transplant. AIM-HIGH stopped early for futility at 1,500 to 2,000 mg of extended-release niacin. HPS2-THRIVE found a significant excess of serious adverse events at a similar dose in 25,673 patients.
Every one of those studies used a pharmacological dose of niacin, two hundred to three hundred times the 7.5 mg NE on this panel, prescribed to alter blood lipids. That is a drug use of a vitamin, and it is regulated as one.
